Each semester, students registered for performance instruction perform for a faculty committee during the exam period. Exam critiques and semester reports provide a written evaluation of progress and suggestions for improvement.
Click on the link below for the form needed. PDF forms may be filled in on a PC or Mac and then printed, signed, and photocopied; they may also simply be printed and photocopied as blank forms. These files cannot be saved as a separate document. The Microsoft Word documents can be edited by the user as needed and saved as a new document.
